Simultaneous Bilateral Total Knee Replacement Versus Unilateral Replacement



Abstract
We performed 20 simultaneous bilateral total knee arthroplasties(40knees) and k31 Unilateral arthroplasties from April 1989 to March 1993 and the two groups of patients were evaluated looking at the length of hospital stay, complications, blood
loss and
outcomes.
The average follow up period over 16 months. The patients in both groups were primarily osteoaarthritic(32cases) or rheumatoid arthritic(19cases) with an average age of 60 years. Most case of femur(97%) were non-cemented and tibia and
patellae(85%)
were
cemented. The prosthetic devices included 48 P.F.C., 15 A.M.K. and 8 Genesis. The average length of stay was similar with the bilateral length of stay being 31 days and unilateral 28days. The complication rate in the bilateral series was not
increased
over the unilaterals. The average time in the operating room was 3.25 hours on the bilateral cases and 1.83 hours on the unilateal cases. The clinical result were likewise no different with the postpoerative HSS scores in the bilateral averaging

and
86 in the unilaterals.
The average flexion postpoeratively was 109 degrees in the bilaterals asversus 113 degrees in the unilaterals. But difficulties in daily life of bilateral cases was much more than unilateral cases.
Theses results support to justify the performance of bilateral simultaneous total knee replacement in patients with bilateral severe arthritis providing surgical team is able to carry out the operative procedure expeditiously.
